Healthy Lifestyle Program

About the Healthy Lifestyle Program

The Cockburn Healthy Lifestyle program aims to help people of all ages to achieve long-term health. The program is open to people who have concerns about their weight or lifestyle.
Following an initial health check, participants are supported and guided through a tailored program.
Depending on your needs, the program may include guidance on:
  • Weight management
  • Diet and nutrition education
  • Physical activity and fitness
  • Mental health
  • Child-specific weight and obesity (see OWL program below).
Participants also receive free ongoing support from a dedicated program coordinator.
The program is run through Cockburn Integrated Health (CIH) and is supported and sponsored by the City.

The Optimal Weight for Life program was developed to help children and young people who are currently overweight.

The goal of the program is to help children, young people and their families to make lasting changes to support them in reaching their optimal weight and enable better health for life.

The program, supported by the City, is a joint initiative between NursePrac Australia, Fiona Stanley Hospital and Cockburn Integrated Health.

The OWL program includes:

  • Detailed health assessment by a paediatric nurse practitioner and a paediatrician

  • Treatment plan personalised for the child/young person and family

  • Psychological support

  • Heal and wellness education

  • Follow-up clinic visits to help keep on track

  • Ongoing support.

Medicare covers the program and you don’t need a medical referral.
