Please complete this form if you need to report a safety or maintenance issue at the community venue you use if you are:
  1. A sporting club with a seasonal hire arrangement of a City of Cockburn reserve/clubrooms, or;
  2. A regular or casual hire user of a City of Cockburn community facility. 
Please upload up to three (3) photos to assist City staff identify the problem more quickly.

  • Does this issue present an immediate safety risk to people using the facility?
  • Does this issue pose a risk to building security or structure?

Community Venue Maintenance Request – Assessment Criteria

Level 1 - High Priority (within 24 hours)
  • Repair within 24 hours to enable the hirer to safely provide their service to patrons/members.
  • No alternative options to manage the risk.
  • Potential for additional damage, theft or financial loss if issue is not resolved. 
Examples:
  1. Floodlights not working and games taking place at the site within 24 hours.
  2. Hole in ceiling and water is coming through.
  3. Sewerage system is blocked.
  4. Facility hasn’t been found in a clean condition.
Level 2 - Medium Priority (24 hours - 3 days)
  • Booking continues but maintenance item will need to be resolved for the next booking.
  • Safety measures put in place to prevent any risk to users.
  • Alternative options may be available.
  • Chance of additional damage, potential for theft and financial loss if not repaired.
Examples:
  1. Trip hazard in pavement.
  2. Air conditioning is not working.
Level 3 - Low Priority (3 days+)
  • Repair in more than 3 days as hirer can conduct their service safely and without inconvenience to patrons/members.
  • Alternative options are available.
  • Safety measures can be put in place to prevent any risk.
  • No risk of theft, additional damage or financial loss.
Examples
  1. Key/Swipe Card has been lost and requires a replacement.
  2. Toilet seat broken and there are other WC’s with toilet seat.
  3. Ceiling light not working but still sufficient ambient light to see.
  4. Table or chair broken and there are many spares.
