Please note:  Hiring of these materials is only available to resistents and groups in the City of Cockburn.

The hirer named below, agrees to the following Terms and Conditions:

  1. Nominate person(s) to collect and return items from/to the City’s Operation Centre, 52 Wellard Street, Bibra Lake.
  2. Provide at least one week’s notice of collection and drop off times to ensure staff members are available. The hirer is responsible for the pickup and return of all items on time.
  3. Provide, in writing, names, addresses and details of persons responsible for items during the period of hire.
  4. Complete inventory checklist on collection and return to ensure that all items are accounted for, with any damaged items replaced.
  5. Ensure that any damage is reported to the City of Cockburn upon the item’s return.
I have read and understand the above terms and conditions, and confirm that all information submitted in this application to be true and correct.
