Application Process
The person(s) named must be deceased and have a clear connection to the City of Cockburn community.
Complete and submit this form for assessment. Applications are assessed in accordance with City policy.
If approved,
City of Cockburn Fees and Charges

The city will supply and install the plaque (sample below)
Multiple plaques may be installed on the same park bench if demand requires
Once payment is received it will take approximately 3 months to complete
