Registration transfers

If you want to transfer your pet's registration to the City of Cockburn from another council, please visit our Dogs and Cats webpage.

Please complete the form below if your animal has:
  • passed away;
  • had a change of owner;
  • change of address;
  • a new microchip number;
  • been sterilised or;
  • requires a replacement registration tag.
All fields must be completed unless indicated as optional. Your owner information needs to be kept up-to-date.
