Cockburn Community Care services

Cockburn Community Care provides a range of services in the home, community and at the Jean Willis Centre in Hamilton Hill for eligible seniors and people with a disability who reside in Cockburn.

What makes me eligible?

If you are over 65 years of age (over 55 years if you are Aboriginal or Torres Strait Islander) you are eligible to apply for funding for all of our services. All of our services can be purchased privately.

National Disability Insurance Scheme participants over 18 years can access the Social Clubs.

The following schemes provide funding:
  • Home and Community Care - provides basic support in the home and/or community.  Clients are encouraged to work alongside staff.  We can send in a referral for you to the Regional Assessment Service or you can call them directly 1300 785 415.  The Regional Assessment Service team will contact you to make an appointment for their assessment.
  • Home Care Packages - provide a higher level of support for clients with more complex needs.  A comprehensive assessment is performed by a member of the Aged Care Assessment Team - ask your doctor to send a referral in.
  • National Disability Insurance Scheme - provides services to clients under 65 with a disability. Eligibility is determined via an Assessment by Disability Services.

What In-Home services are available?
  • Cleaning
  • Gardening
  • Social support including shopping
  • Respite for carers and family
  • Transport
  • Personal care, medication assistance and meal preparation

What social activities can I get involved in?

Social Clubs at the Jean Willis Centre include:

  • General Group
  • Kwobarup for Aboriginal and Torres Strait Islanders.
  • Intellectual Disability
  • Dementia specific
  • Bingo
  • Outings
  • Transport to and from the Centre and outings

How much does it cost?

Fees are set by the Federal Government and vary across each service program.  Seniors can also attend on a private basis with fees available upon request.
