Cockburn Community Care provides a wide range of care services for frail, aged and younger people with disabilities funded by State and Commonwealth Government initiatives. The service is based at the Jean Willis Centre, on the corner of Healy Road and Ingram Street in Hamilton Hill.
The main office is staffed for community contact between the hours of 8.30am to 4.30pm Monday to Friday.
Cockburn Centre Day Care
There are 2 Centre Day Care Programs operating within the City of Cockburn: Hamilton Hill and Beeliar Community Center.
The Day Centre provides a wide and varied range of activities and outings, which are client focussed to cater to the physical, social and emotional needs of all members. Lunch and refreshments are included in the day program. Transport is provided to and from the Centre in specially equipped vehicles, to allow easy access for people with physical disabilities. i.e. Wheelchair access.
Referrals can be made by a family member, health professional or by the client themselves. A subsidised fee applies and some outings may incur additional costs.

Home Support
The Home Support Services assist people to remain living in their own home in the community. Assistance or support is provided with basic everyday tasks, such as:
Cost for these services are minimal. Fees may vary in accordance with the National Fees Schedule. Please contact the main office number and ask to speak to Vicky Kennedy the Service Coordinator.
Carer Support Service
This Support Service is aimed at those who fit the following definition of a Carer. A Carer is someone who provides care and support for a parent, partner, child, relative, or friend who has a disability, is fail aged, or who has chronic mental or physical illness. (Definition courtesy of Carers WA). Carers are a valued resource in the Community providing most of the in-home care necessary. Without this care, many of those cared for would be in an institutional setting.

Kwobarup Aboriginal Program
This service is aimed specifically at our Aboriginal Community. The Kwobarup program offers a range of services to fail, aged and disabled Aboriginal people. Services include:
At our Day Centre we run a cultural art program, activities and outings in a relaxed and friendly atmosphere.
This is also a good point to contact for more resource information about other Aboriginal specific services provided in the community. Fees for this service will be determined at the time of assessment. Please contact the main office number and ask to speak to Sue Lewis the Service Coordinator or Dorothy Petersen Community Health Worker.
Community Aged Care Packages
The Community Aged Carer Packages are a Commonwealth Government funded scheme. These are planned and coordinated packages of care to assist people to remain living in their own home. The packages are specifically designed to cater to the individual and are flexible to meet varying needs eg:
To be eligible to receive a Care Package, the client must be assessed by an approved assessment authority; in most cases this will be a hospital based Aged Care Assessment Team (ACAT) A referral for this must be obtained from your doctor.
These services are subject to fees. Unlike other programs within Cockburn Community Care, this service does have an after hours call out facility for emergencies only. Please contact the main office number and ask to speak to Tracey Ramshaw Service Coordinator.
